Our services include caring for people with learning disabilities and those with dementia. We offer a flexible service that puts the person at the heart of what we do, creating care plans tailored to their needs.

Caring for people with learning disabilities

We currently provide care to 1,400 adults with learning disability or associated complex needs. We offer a service to people with Personal Budgets and those funding their own care. We’re members of the Association for Real Change, which champions the development of policy, best practice and training in the field of learning disabilities.

Person-centred planning

We don’t do things ‘for’ customers, we do things ‘with’ them. This includes working with the customer, their friends and their family to put together a support plan that reflects the individual’s needs and wishes. We have lots of experience with:

  • Supported living services – through these, we help customers move towards a range of identified goals and outcomes to enjoy an ordinary lifestyle within their community
  • Housing-related support – each customer’s support plan aims to find the best way to help them independently manage their own home
  • Community-based services – our extensive portfolio of services ranges from support with managing personal care to help with finding a job

Caring for people with dementia

We run specialist dementia services for care commissioners, where our care workers have specific training in the different types of dementia and the symptoms and frustrations that people with dementia experience. We also run a rapid intervention dementia service, which aims to avoid residential care or hospital admission.

We focus on the person, not the dementia, and provide a service that prioritises enablement and independence. By working with each person, understanding their personal history and learning all we can about them, we can tailor a care package that’s rooted in their experiences and relevant to their life. At the heart of our work are the human confidences and shared personal insights that allow us to discover better ways for people to live the life they know, where they’re most at home.
